Convert CEUs To Clock Hours
How to Convert Continuing Education Offerings to Clock Hours
Complete the form below and upload the following documents:
- Copy of course description.
- Certificate of Completion or Certificate of Attendance.
- Complete Professional Development Evaluation form. Include your name, address, Area #, phone number and email address with the forms.
The Clock Hour Chair will review the information and provide you with a Clock Hour Form. This form will need to be completed in full by the member, then returned to the Clock Hour Chair along with payment of $3.00 per clock hour. Make checks payable to SNOW.
When documentation and payment are complete, the Clock Hour Chair will return the completed form to member.
Conversion of Continuing Education offerings to Clock Hours is limited to 3 years prior to the request.
